EMPLOYEES OPEN ENROLLMENT                   (RETIREES CLICK HERE)

The Office of Human Resources is pleased to announce the annual open enrollment period, October 12 through October 23, 2009. During this time, employees can make changes to participation in various insurance and benefit programs (medical, dental, AFLAC and the Section 125 pre-tax program). We also have several new plans to offer this year.

Changes made during this period will be effective January 1, 2010. Remember, this is the only time you are able to make changes to your benefits during the plan year, unless a qualifying event occurs which permits a change (i.e. marriage, birth of child, loss of previous coverage, etc.).

This web page is designed to provide employees with detailed information regarding Edison State College benefit offerings. For additional information, we encourage you to participate in one of the informational meetings scheduled.

INFORMATIONAL MEETINGS

In the next few weeks, Human Resources will be holding informational question and answer sessions regarding the new health and vision insurance plans being offered, changes effecting our current health insurance plans, and other open enrollment benefits. Meetings are scheduled as follows:

Lee Campus
Monday, October 12, 2009 from 2:00 pm to 3:30 pm Taeni Hall (Building S) Room S117
Tuesday, October 13, 2009 from 9:00 am to 10:30 am Taeni Hall (Building S) Room S117
Monday, October 19, 2009 from 2:00 pm to 3:30 pm Taeni Hall (Building S) Room S117

Charlotte Campus
Wednesday, October 14, 2009 from 10:00 am to 11:30 am, 1:00pm to 2:30pm Student Activities Hall (Building SA) Room SA117
A Benefits Specialist will be available on the Charlotte campus all day to process benefit changes.

Collier Campus
Thursday, October 15, 2009 from 10:00 am to 11:30 am, 1:00pm to 2:30pm Building J Room J103
A Benefits Specialist will be available on the Collier campus all day to process benefit changes.

CURRENT PLANS

Plan A - PPO - Plan Overview | Summary of Benefits | Monthly Rates
Below are the plan changes. See rate information for rate changes.
1. Wellness benefit increased from $250 to $500.
2. Plan covers 100% of the Allowed Amount for one time routine colonoscopy for ages< 50+.
3. Inpatient rehabilitation decreased from 30 days to 21 days


Plan B – HMO - Plan Overview | Summary of Benefits | Monthly Rates
There are no plan changes to the BlueCare HMO health plan.

Plan C – Hospital Indemnity - Summary of Benefits | Vision Exclusions
Below are the plan changes.
1.The dental and vision plans will change from Blue Cross and Blue Shield of Florida to Metropolitan Life. Information will be sent to Plac C participants by MetLife.
2.Employees enrolled in the Hospital Indemnity (Plan C) health plan will have the opportunity to waive coverage if they choose.


Plan D – Health Reimbursement Account - Plan Overview | Summary of Benefits | Monthly Rates
Below are the plan changes. See rate information for rate changes.
1. Wellness benefit increased from $250 to $500.

Dental Plans - Plan Overview | Monthly Rates
The dental plans will remain the same for 2010 with a rate decrease of 14%. Please see rate information.

This health plan is available to benefit eligible employees and their families.


Plan F – Dependent Only PPO - Plan Overview | Summary of Benefits | Monthly Rates
This plan is available to benefit eligible employees interested in an alternative option to cover their dependents.


Vision Plan - Summary of Benefits | Monthly Rates
The College is introducing a Vision Plan through MetLife.
