2013 HEALTH PLAN OPTIONS
Edison State College offers medical insurance through Florida Blue to all regular full-time employees. A variety of health plans are offered. The premium cost for the employee is paid by the college, employees are responsible for the premium cost of dependent coverage if elected. Employees and dependents covered through Florida Blue have access to the Member Services program which allows members to review claims, order replacement cards and view the Provider Directory. Medical insurance is effective the first of the month following date of hire and premiums are paid one month in advance.

Dental Plans Plan Comparison Monthly Rates
Full-time employees may elect dental insurance for themselves and their dependents through Delta Dental. Two dental plans are offered, Delta Dental PPO Plan and Delta Care USA (HMO) Plan. Dental insurance is a voluntary benefit paid for by the employee. Coverage is effective the first of the month following date of hire and premiums are paid one month in advance.
Vision Plan Plan Summary Monthly Rates
Full-time employees may elect vision insurance for themselves and their dependents through VSP Vision plan. This is a PPO Vision plan, so in and out of network benefits are available. Informational booklets and enrollment forms are available in Human Resources or you can view additional information at www.vsp.com. Vision is a voluntary benefit paid for by the employee. Coverage is effective the first of the month following date of hire and premiums are paid one month in advance.
Flexible Spending Account - Plan Summary
Would You Like To Reduce Your Out of Pocket Medical Cost, Reduce Taxes and Increase Your Take-Home Pay?
Enroll in the Flexible Spending Account (FSA). There is no cost to participate in the FSA, however, employees who elect the Debit Card option will be charged an annual fee of $12.00.
Medical Reimbursement Account An IRS approved plan that allows employees to set aside money on a Tax Free basis to pay for Medical Expenses. Some of the Medical Expenses that are eligible to be reimbursed include: Deductibles, Co-payments, Prescriptions, Mental Health Care, Orthodontist, Dental Expenses, Chiropractor Services and Vision Expenses including eye exams, glasses and contacts.
By paying for your out of pocket Medical expenses with Tax-free dollars, the average employee will save approximately 23% to 25%. Also, employees have access to the money they put in the FSA before it is taken out of your paycheck. For example, if you elect to put $600.00 into the FSA, you have access to the $600.00 on January 1st, before any deductions are withheld.
Dependent Care Account Allows you to pay for daycare expenses for eligible children and adults. Qualified expenses include adult and child daycare centers, preschool and before/after school care. Employees who enroll in this plan are able to pay daycare expenses with Tax Free income resulting in savings of approximately 25% of the daycare costs.
AFLAC Cancer Indemnity Plan Monthly Rates
Full-time employees may elect the Cancer Indemnity Plan offered through AFLAC for themselves and their dependents. This is a voluntary benefit paid for by the employee. The plan is designed to be a supplement to major medical insurance. All benefits are paid directly to the participant and cover expenses not usually paid for by major medical plans (i.e. lodging and travel to/from a cancer treatment center). The plan also reimburses the participant up to $75/year for cancer screenings. For more information contact Randy Powers at (239)-540-2101 or randolph_powers@us.aflac.com. Life Insurance Certificate of Group Life
The College provides a $15,000 basic term life insurance policy through Unum Group Life, including accidental death and dismemberment, for all full time regular employees at no cost to the employee. Coverage is effective the first of the month following date of hire. In addition, employees can purchase supplemental life insurance at 1, 2, or 3 times their base salary. The cost is $.277 per $1000.00 of coverage per month. Supplemental life insurance requires application and approval from the insurance company. Supplemental Life Insurance applications are available in the Human Resources office. Coverage effective date is determined by the insurance company and premiums are paid one month in advance.
Long-Term Disability Insurance
Edison State College provides long-term disability insurance for all full-time employees at no cost to the employee. Eligible employees receive benefits on the 91st day of disability.
Short-Term Disability Insurance
This benefit is available to regular full time employees within the first 30 days of employment. New full-time employees may elect short-term disability through the Standard Insurance Company. Benefits for eligible employees begin on the 31st day of disability and end on the 90th day. The monthly cost is $.292 per $100.00 of monthly salary. Coverage is effective the first of the month following date of hire and premiums are paid one month in advance. STD enrollment forms are available in the Human Resource Office.
